Technical Assistance:
Until there is a completed CRC and CPS for the employee who is missing the forms, the employee can not work as he has been employed for more than 3o days.

Comments:
An unannounced monitoring inspection was conducted on 3/21/16 between 10:15 and 1:00. The inspection was conducted with the director. The inspector spoke with staff and children in all classrooms. The following staff/child ratios were observed: Infants ? two staff with seven children; infants & toddlers - one staff with four children; Toddlers - one staff with five children, 2?s - three staff with 23 children; 3?s - two staff with 20 children; 4 and 5?s - three staff with 23 children. The children were engaged in various activities including eating lunch, reading and making Easter eggs. Lunch today was grilled chicken nuggets, lima beans, apple sauce, bread and milk. Health inspection ? 4/29/15; Fire inspection ? 4/3/15 Last fire drill 2/17/16 and a shelter-in-place drill was conducted on 2/25/16 All required documents were posted including license, licensing notices, menu, and evacuation information.

Standard #: 22VAC40-185-160-A
Description: The center failed to obtain a TB screening in the required time frame for a new staff person. Evidence: A review of the file record for Staff #1 found that she began working on 7/29/15 and the TB screening was dated 9/15/15, Documentation of a negative TB screening is required within 21 days of employment.

Plan of Correction: Going forward will make sure it's received within the required time frame,

Standard #: 22VAC40-185-270-A
Description: The center failed to maintain the playground equipment in a clean and safe condition. Evidence: A check of the playground found that the yellow climbing equipment located on the school age playground had large areas of peeling paint.

Plan of Correction: work order submitted to have equipment sanded and repainted - 1 month

Standard #: 22VAC40-191-40-D-1-B
Description: The center was unable to locate the completed State Police Criminal Record Check (CRC) and Social Services Child Protective Services Registry check (CPS) for one new employee. Evidence: A review of the record for Staff #1 found that there was no completed CRC or CPS for the employee. The employee began working on 7/27/15. The completed CRC and CPS are required by the 30th day of employment. The director stated that the assistant director had been through the records and had pulled information. The pulled information had not been re-filed and could not be located. The employee is on leave this week.

Plan of Correction: Will send to licensing once my asst. director come in. He is on spring break & not currently working this week. If not able to locate new forms will be submitted - 1 week
