Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

The inspector will send example documents. Discussed that a central registry check is required when a household member turns 14 years old.

Comments:

An unannounced, on-site monitoring inspection was initiated on and completed on 6/10/2025. The on-site inspection began at 9:50am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 24. The inspector reviewed five children?s records and five caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-2
Description:	Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #2, who had been employed for two months, did not have a completed national criminal background check.
Plan of Correction:	The provider will obtain criminal background checks for the staff member.
Standard #:	8VAC20-800-200-A
Description:	The provider must orient assistants by the end of their first week of assuming job responsibilities. Staff #2 provided care for children for two months without receiving orientation.
Plan of Correction:	The provider will orient the staff and keep record of orientation in their file.
Standard #:	8VAC20-800-200-B
Description:	Assistants' and substitute providers' records shall contain two or more references as to character and reputation and competency. References must be checked before employment. There was no record of references being checked for Staff #2.
Plan of Correction:	The provider will ensure that two references are completed before hiring staff.
