22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 11:30 p.m. to 1:00 p.m. At the time of entrance the following adult to child ratios were observed: 9 school aged children with 1 staff 7 children, 4 to 5 years old, with 1 staff 9 children, 3 years old, with 1 staff (napping) 9 children, 2 1/2 to 3 years old, with 1 staff (napping) 9 children, 17 months to 2 years old, with 1 staff (napping) 3 infants, 8 weeks to 21 months old, with 1 staff The records sample size consisted of 3 children?s records and 2 staff records. Children were observed resting quietly on mats during nap time, eating lunch which consisted of spaghetti with meat sauce, salad, apple sauce and milk, playing table games and watching a children's television program, playing outdoors, and leaving for a planned outing to the park. Infants were observed playing on the floor, sleeping on cribs and being held and bottle fed. Medication administration was not observed. However, 2 emergency medications stored on site were reviewed with regard to storage, labeling, written authorization and required documentation. The van used to transport children was viewed as well as required supplies required during transportation. Areas of non-compliance are identified on the violation notice. The licensee provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.
Standard #: 22VAC40-185-510-A Description: Based on a review of medication, the licensee did not ensure that prescription medication is only administered to children with a parents written (signed) authorization. Evidence: 1. Two prescription medications (Albuterol and Epipen Jr.) were kept on sight for emergency use. 2. The written authorization form for the Epipen Jr. was completed, but was not signed by the child's parent. Plan of Correction: The administrator had already flagged the form with a sticky note to have it signed by the child's parent and states that she will have the parent sign it this afternoon when she picks up the child.
