During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 6/3/2025. The on-site inspection began at 1:07pm and ended at 1:55pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 8. The inspector reviewed 3 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/27/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-3 Description: The provider shall obtain the results of a child abuse and neglect central registry staff for all household members age 14 and older.
Household Member #1 is older than age 14 and did not have the results of a central registry search on file.Plan of Correction: At the moment they are out of town. They will return next week and I will ensure their central registry check is completed and submitted promptly immediately upon return. I understand the importance of maintaining compliance with all regulatory requirements and will make this top priority upon their return. Thank you for your understanding.
Standard #: 8VAC20-800-60-A Description: The provider shall maintain an up-to-date record at the family day home for each enrolled child.
Three children were present at the time of inspection and did not have records on site. The children have been in care for one to two days.Plan of Correction: This issue has been corrected. All required records for 3 children are now complete and available on site as of 6/04/25.
Standard #: 8VAC20-800-90-A Description: The provider shall obtain documentation that children have been adequately immunized before they may attend the family day home.
Child #1, Child #2 and Child #3 have attended the program for one to two days and did not have immunizations on file.Plan of Correction: This issue has now been corrected. All required immunization records are currently on file and in hand as of 06/04/25.
Standard #: 8VAC20-800-520-C Description: Cribs shall not have mesh sides.
Child #3, age 12 months, was observed in a pack and play crib with mesh sides.Plan of Correction: This is to confirm the crib with mesh sides identified during inspection has been removed immediately from the daycare facility.
