Discussed the following standard: staff-to-children ratio requirements.
An unannounced complaint investigation was conducted on 03/09/2016 from approximately 7:30am to 9:00am in response to allegations received by our office regarding staff-to-children ratio requirements, and medications. Based upon observations made, multiple staff interviews, and documentation reviewed, the preponderance of evidence supports the allegation regarding staff-to-children ratio requirements; therefore, the complaint is deemed to be valid. Standard #: 22VAC40-185-350-E-2 Complaint related: Yes Description: Based upon multiple staff interviews and documentation reviewed, ratio requirements of one staff member for every four children was not met for a child under 16 months in the Toddler's classroom. Evidence: 1) Staff interviews confirmed that the Toddler's classroom did not meet the required one staff member for every four children during today's inspection when a child under the age of 16 months was in care (Child A; 14 months). The staff-to-children ratio requirements staff were applying was one child for every five children; exceeding the ratio by 2 children. 2) The Child Supervision Record (CSR) document confirmed that on 03/07/2016, there were 5 children, including Child A (14 months), under the supervision of 1 staff member from approximately 8:05am to 8:20am, exceeding the ratio by 1 child. At approximately 8:20am, there were 6 children under the supervision of 1 staff member, exceeding the ratio by 2 children. Plan of Correction: Not available online. Standard #: 22VAC40-185-350-E-4 Complaint related: Yes Description: Based upon observations, documentation reviewed, and multiple interviews, ratio requirements of one staff for every ten children was not met in the Pre-K classroom. Evidence: 1) Multiple staff interviews confirmed that every morning the Preschool A, Preschool B, and Pre-K children are combined until approximately 8:00am. Additionally, staff interviews confirmed that on 03/07/2016 and during today's inspection, the staff-to-children ratio requirement of one staff for every ten children was not met. 2) The CSR documentation confirmed that on 03/07/2016, there were 23 children under the supervision of 2 staff members, exceeding the required ratio by 3 children from approximately 7:42am to 8:00am. 3) During today's inspection, upon arrival to the Pre-K classroom, at approximately 7:30am, there were 13 children under the supervision of 1 staff member, exceeding the ratio requirement by 3 children. 4) Staff interview confirmed that during today's inspection, Staff A was asked to stand between the Pre-K and School Age classroom and supervise both classrooms while Staff B went to get help in front office as they were 13 children present. At approximately 7:25am, there were 9 children in the School Age classroom, and 13 children in the Pre-K classroom. A total of 21 children were under the supervision of Staff B for less than two minutes, exceeding the ratio requirement by 11 children. Plan of Correction: Not available online.
