Comments:
Your time and assistance were appreciated during the unannounced monitoring inspection conducted on March 29, 2016, between the hours of approximately 10:18 a.m. and 1:35 p.m. Twenty-three (23) children were enrolled and twenty-one (21) children were in the care of four staff members, in three groups, during the on site inspection. One additional staff member was present and assisted, as needed. Three children's records were reviewed and four staff records were updated. One child's medication and accompanying documentation was reviewed. Posted information (license, licensing inspection documentation, emergency evacuation diagram, emergency telephone numbers, menu and classroom schedule), safety equipment and available documents (emergency evacuation drill and shelter-in-place drill records, insurance and local agency reports) were reviewed. The daily schedule accurately reflected the activities provided. Lunch was provided by the children's parents. Program observation was completed. Activities observed on this date included: indoor play with classroom materials/toys and learning activities (group game, painting caterpillars, a matching game and a science experiment - dissolving egg shells); outdoor play; diapering, restroom and handwashing procedures; lunch; stories; and naptime. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Children's artwork was displayed. Areas of the licensing standards discussed included: sanitizing procedures for tables used for eating, labeling and dating of lunch boxes and a system for documenting staff information required to be periodically updated. The exception to compliance was noted on the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on update of three staff records and discussion with staff #1 on March 29, 2016, one staff member did not obtain and submit the results of a follow-up tuberculosis (TB) screening/exam report at least two years from the date of the first initial screening/testing. Evidence: 1. The date of staff #1's initial TB screening/exam was 11/01/2013. An updated screening/exam report was not on file. 2. Staff #1 verified that an updated TB exam/screening had not been obtained.

Plan of Correction: An updated TB exam/screening will be obtained. In the future, TB exam reports/screenings will be obtained and the results submitted within two years of the date of the employee's first initial exam/screening. Administrative staff will develop/implement a system to ensure updates are obtained in a timely manner.
