63.2 Child Abuse and Neglect
An investigation was conducted on 2/10, 2/19, and 2/24/2016 in response to allegations received by our office regarding supervision and care and protection of children in care. Based upon multiple interviews the preponderance of evidence supports violations and the complaint is deemed to be valid. An exit interview was conducted with the center Director and corrective actions and staff training had already been implemented by center administration by the close of the investigation. Standard #: 22VAC40-185-340-F Complaint related: No Description: Based on multiple interviews, staff failed to keep children under the age of 10 years of age within actual sight and sound supervision of staff at all times. Evidence: While in the summer camp/ after school classroom ( on an undetermined date) staff A and B were not aware that Child A (acting in the role of table captain) sent two children to the rest room without notifying a teacher first. Child A was over heard by Child B using sexually explicit language in the rest room area with Child C and Child D. Child B then reported the incident to staff A. Staff A confirmed that they did not see or hear the children in the rest room and when told by child B they went to check on the children and everything was OK. Plan of Correction: Correction: During School Age Camps we will implement Junior camp and Senior Camp based on age and development level of each child for part of the day effective immediately. School Age children will continue to ask permission to use the rest room. A staff member will ensure that no more than 1 school age child is using the boy?s rest room at one time effective immediately. School age staff will keep a log of identified interactions and what was done. School age staff will document on incident reports and or classroom log book, any use of inappropriate language or actions not appropriate for that age level and notify parents effective immediately. School Age children will not have table captains, effective immediately. All staff will attend refresher training on sight and sound supervision before summer camp begins.
