Comments:
53 Children were in care during the inspection, The children were observed resting, eating snack and free play. Six staff and children's files were reviewed. Sample of medications were reviewed. Inspection time 1:30pm-3:30pm

Standard #: 22VAC40-185-60-A
Description: Based on review of six children's records not all were complete. Evidence: Child A with a start date of 12/1/15 was missing the house address for two emergency contact and work number for father. Child B with a start date of 6/25/14 was missing the physician's phone number. Child C with a start date of 6/25/15 was missing the work number for the father. Child D was missing the address for one emergency contact.

Plan of Correction: I will ask the parents for the missing information;

Standard #: 22VAC40-185-450-A
Description: Based on observation not all resting children had a top and bottom cover. Evidence: One child in the Pre-K room was resting on a cot without a top cover.

Plan of Correction: The director got the child a cover.
