An unannounced monitoring inspection was conducted in the center during the after-school program. There were 12 children present, their ages ranged from 5 to 12 years. As reported, 22 children are currently enrolled. Two staff were supervising the children's activities. The children were observed in the school cafeteria during varied activities which included: table top games, homework time, and snack time. The staff were engaged with the children and provided guidance when needed. Dismissal procedures were observed at the end of the program day. A sample of 4 children's records were reviewed. Two partial staff records were reviewed. Documentation of the annual fire and health inspections was current. Findings of the inspection were reviewed with the Site Director at exit. The Acknowledgment of Inspection was signed and left for posting in the center. The on-site inspection was conducted from 3:00 p.m. to 5:30 p.m. A plan of correction was provided by the Child Care Coordinator. Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the Center failed to ensure required information was complete in each child's record: name, address, phone number of two designated people to call in an emergency if a parent cannot be reached. Evidence: A sample of 4 children's records was reviewed at the time of inspection. The LI also reviewed 1 additional child's record to verify information was completed as cited at the last inspection. Complete physical addresses of emergency contacts were not available for review at this inspection for child a. Plan of Correction: The Site Director will make sure that all paperwork is filled out completely.
Standard #: 22VAC40-185-510-C Description: Based on the LI's observations, the Center failed to ensure medication administration procedures were followed: (3.) include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent.; (4.) include methods to prevent use of outdated medication. Evidence: The LI reviewed medications and the written parental authorization for child a. An over-the-counter medication to be administered as needed had a manufacturers' expiration date of 01/2016. The parental authorization did not indicate a start or end date for the medication administration. Plan of Correction: The Childcare Coordinator will create a medication log for staff to keep posted in their cabinets at the site. This will ensure that staff will let parent know 30 days before medication expires that they need a current medication. MAT trained personnel will make sure all paperwork is filled out correctly.
Standard #: 22VAC40-185-510-E Description: Based on review of an over-the-counter medication, the Center failed to ensure the dosage amount was labeled on the medication by the child's parent. The manufacturer's instructions noted to administer 1-2 tsp. Evidence: The LI reviewed an allergy medication to be administered as needed in an emergency (child a.). The dosage amount was not indicated on the medication box. Plan of Correction: Childcare Coordinator - will create a medication log for staff to keep posted in their cabinets at the site. MAT trained personnel will make sure all paperwork is filled out correctly.
