Discussed administration of rectal medication.

Comments:
Licensing Inspector arrived at 10:25 AM and departed at 12:45 PM to conduct a monitoring visit. Upon arrival, the facility was having a lockdown drill. At the time of the tour of Rainbow Station, there were 92 children, ages infant through preschool, in care with 16 staff. Children were observed reading, working at science table, playing in home living and eating lunch. There were no children in care in the Get Well Place. At the time of the tour of the Village, there were 48 school-age children in care with five staff. Village children were observed in centers including but limited to library, theater, gym and playing video games. Violations were found in the areas of physical plant and special care provisions & emergencies. Medication, first aid and injury reports were also observed. The sample size was twelve staff and eight children's records. An exit interview was held at the conclusion of the inspection and the facility's responses were received at that time.

Description: Based on observation, it was determined that the facility did not ensure that each restroom area provided for children was equipped with soap, toilet paper and disposable towels. Evidence: 1. There are two stall toilets in the Village area that are used for male school-age children. One of the toilets was not functioning and there was not toilet paper available in the other stall. 2. There are four stall toilets in the Village area that are used for female school-age children. There was no toilet paper available in three of the four stalls.

Plan of Correction: Toilet paper was placed in stalls during the inspection. Staff will be retrained to check restrooms on a regular basis to ensure that there are toilet paper and paper towels available in the classrooms.

Description: Based on review of three diaper preparations in the infant classroom, it was determined that the facility did not ensure that they obtained written parental authorization for diaper ointments they applied. Evidence: One of three diaper ointments did not have written parental authorization.

Plan of Correction: Center staff will obtain written authorization when the parent picks up. Staff will be reminded to have parents give written authorization when leaving diaper ointment.
