Reminder to get a new Epi-pen for child #5, since it has expired.

An unannounced renewal inspection was initiated today at 10:30am. The physical plant, 4 staff files, 4 children's files, annual fire inspection, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 14 children under the supervision of 4 staff. During the inspection, children were observed preparing for and going outside to enjoy free play and being helped with toileting. For lunch, the children bring their lunches from home daily. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, 2 out of 4 children's files do not have additional immunizations once every six months for children under the age of two years. Evidence 1. Child #1 is 21 months old. The most recent immunization record on file was completed at 1 year old. 2. Child #3 is 26 months old. The most recent immunization record on file was completed at 19 months old.

Description: Based on observations, the center did not follow all procedures for administering medications. Evidence 1. Child #4's written authorization for an epi-pen was effective until January 1, 2016.

Description: Based on observations, the center did not document a practice evacuation drill for each month. Evidence 1. There was no documentation of a drill having been completed in March 2016.
