Comments:
An unannounced monitoring inspection was conducted on 04/13/2016 from 9:15am - 11:00am. During the inspection there were 28 children ages five months to four years old in care with seven staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms. Records were reviewed for five children and three staff. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of three staff records, it was determined that the facility did not ensure that each staff member has documentation of a negative tuberculosis screening within 21 days after beginning employment. Evidence: 1. The record for staff #1 (date of hire 3/14/16) did not contain documentation of a negative tuberculosis screening. 2. Staff #4 (Program Director) confirmed that staff #1 did not have documentation of a negative tuberculosis screening.

Plan of Correction: The facility responded: Once staff member #1 returns from being sick I will be sure to have her get a current TB screening.

Standard #: 22VAC40-185-550-M
Description: Based on a review on the facility's injury/accident reports, it was determined that the facility did not ensure that the written record of children's serious and minor injuries included all required items. Evidence: 1. Three of three injury reports reviewed did not contain the future action to prevent recurrence of the injury. 2. Staff #4 (Program Director) reviewed the three injury reports confirmed that the injury reports did not include all the required items.

Plan of Correction: The facility responded: I will add future prevention actions to the accident reports.
