Comments:
The licensing inspector and the license administrator conducted an unannounced monitoring inspection March 17, 2016 from approximately 10:05am to 12:15pm to monitor the licensee?s provisional license status. The following ratios were observed with a total of 14 children present during the inspection: Toddlers & Twos - 9 children with 2 staff Preschool - 5 children with 1 staff School Age children were not present during the inspection. During the inspection, children and staff were observed participating in a variety of activities. Children were observed reviewing shapes and colors, playing with toys, drawing St. Patrick?s Day pictures, taking bathroom breaks and preparing for lunch. During the inspection, three children?s and four staff records were reviewed. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on review of three children's records, one child did not have a current immunization record before attending the center. Evidence: During the inspection, the record for Child #2 (enrollment date 02/01/16) did not contain a current immunization record.

Description: Based on review of three children's records, one record did not have a physical examination obtained within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Child #2, 15 months of age, (enrollment date 02/01/16) contained a physical exam dated 06/09/15. Children age seven months through 18 months are required to obtain a physical exam within three months prior to attendance or within the first 30 days of attending the center.

Description: Based on review of four staff records, one staff record did not contain a tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 02/01/16) did not contain documentation of a negative tuberculosis screening. Staff are required to submit documentation of a negative tuberculosis screening completed within 12 months prior to or within the first 21 days of beginning employment.

Description: Based on review of four staff records, two staff records did not contain an updated negative tuberculosis screening. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 07/15/13) contained a tuberculosis screening dated 07/17/12 and the record for Staff #3 (hire date 09/03/13) contained a tuberculosis screening dated 10/15/13. Staff are required to submit a negative tuberculosis screening at least every two years from the initial screening.

Description: Based on review of four staff records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 02/01/16) did not contain two documented references prior to the staff member beginning employment. The record for Staff #4 (hire date 01/15/16) did not contain the documentation of education for a program leader. Staff #4 was identified as the program leader for the Preschool group.

Description: Based on observation, the center did not ensure all equipment was maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed a cracked outlet plate located in the Preschool play area.

Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by the children. Evidence: During the inspection, the licensing inspector observed a container of Lysol and a spray bottle of bleach water sitting on top of an approximate four foot tall cabinet. During the inspection, a staff member placed the containers into the cabinet and locked it.

Description: Based on observation, the center did not ensure cleaning and sanitizing materials were stored in an area physically separate from food. Evidence: During the inspection, the licensing inspector observed a container of Lysol and a spray bottle containing bleach water sitting on top of a cabinet beside an opened packet of Pop-Tarts.

Description: Based on observation, the center did not maintain the required staff-to-child ratio for the youngest child present in the group. Evidence: During the inspection, the licensing inspector observed nine children being supervised by two staff members in the Toddler/Twos classroom. One of the children present in the group was 15 months of age. The required staff-to-child ratio was one staff member for every four children present. Three staff members were required to maintain the required staff-to-child ratio.

Description: Based on observation and interview, the center did not ensure emergency evacuation and shelter-in-place procedures were posted in a location conspicuous to staff and children. Evidence: During the inspection, the licensing inspector observed the emergency evacuation and shelter-in-place procedures were not posted. During the inspection, the owner stated the emergency procedures were taken down to make a copy of them. The procedures were re-hung during the inspection.

Evidence: During the inspection, the licensing inspector observed the displayed fire drill log did not have a documented monthly practiced evacuation drill for the month of February 2016.
