Technical Assistance:
Discussed infants and older children being in separate areas. Education verification for lead qualified staff was discussed.

An unannounced renewal was conducted from 9 am - 1145. The Inspector walked through all rooms and observed ratios, program, and staff interaction with the children. The staff were able to answer questions regarding the care of the children and any special care provisions. The Inspector reviewed staff records, medication, annual reports/permits, and attendance records. The Inspector also observed hand washing, bathroom/diaper changes, and outside play. Description: Based on record review and staff interviews, the annual update of the Injury Prevention plan is not complete. Evidence: 1.Records have been organized by month but the monthly updates were not included. 2. Staff stated discussions have been put in place for monthly review with staff but did not have in writing.

Plan of Correction: We will locate the previous completed updates and continue to make sure monthly updates are included.

Description: Based on observation there are two items inside not maintained. 1. Finger guard leading into Pre K room is cracked in two places and not securely attached on one part of door. 2. The vinyl floor in girl's bathroom is cracked around the drainage spout and the drainage is sunk below the floor.

Plan of Correction: We will put in work orders in immediately. We can get finger guard corrected within the day but will need to address the floor again.

Description: Based on observation and record review, the mixed age group was over ratio by 1 child during the morning inspection. Evidence: 1. At 9 05 AM, the inspector entered the mixed age group class and the two staff had 17 children. The staff was asked about the age of the children and they were 2 years- young 4 years. 2. The two's ratio is 1 to 8.

Plan of Correction: We were short one staff due to a personal situation but will continue to work with staff on communication as we stay on top of ratios.

Description: Based on record review and staff interviews, the Infant room was over ratio this morning, by 3, from 830-910. Evidence: 1. There were 12 children present at 925 with 3 staff. The daily head count sheet documented that there were 11 with 2 staff. 2. Interviews with staff acknowledged there were 2 staff with 11 infants from 830 until the third person arrived 910.

Plan of Correction: Infant care was unusual today by three children who do not usually arrive so early. We will continue to monitor the situation.

Description: One prescription medication did not have the original prescription label attached.

Plan of Correction: We have been working with the parent to get the label as this was a refill because the last medication had just expired.

Standard #: 22VAC40-80-120-E-2
Description: The center did not have the most recent Violation Notice posted. The Inspection Summary was posted but the Violations were not in an area for the public to view.

Plan of Correction: We will post in the future.
