During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on July 11, 2025. The on-site inspection began at 11:25am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 7. The inspector reviewed 3 children?s records and 1 caregiver record on-site This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 24, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation

Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency and written authorization for emergency medical care.

The record for child #1 was missing the address for one emergency contact; and did not have a second emergency contact. The child was present during the inspection and had been in care for five months.

The record for child #2 was missing authorization for emergency medical care. The child was present during the inspection and had been in care for over one year.

Plan of Correction: We have updated the children's records with the missing information.

Standard #: 8VAC20-800-70-B
Description: Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.

The record for child #2 did not contain all of the required parent acknowledgements regarding family day home operations and administration. The child was present during the inspection and had been in care for over one year.

Plan of Correction: We have updated the child's records with the missing information.

Standard #: 8VAC20-800-690-D
Description: The diapering surface shall be: Nonabsorbent and washable.

Two small tears were observed on the diaper changing pad making the surface absorbent and not washable.

Plan of Correction: A new diaper changing pad has been ordered.
