Discussion was held on the topics of current enrollment and current staffing.

Comments:
The following inspection took place between the hours of 10:30 am through 2:30 pm. Today, I met with the Staff of Chesterbrook Academy #801 located in Fair Lakes, Virginia. At the start of this unannounced renewal inspection there were a total of 116 children with 21 staff; good ratios in all 7 rooms. A total of 5 children?s files and 7 staff files were reviewed. The children were engaged in the following: art, center activities within each room, large motor skills on the playground, story time, infant feeding, tummy time, diaper changing, hand washing, lunch, nap time, games, and other organized activities. Standard #: 22VAC40-185-160-A
Description: Based on review, 2 of 7 staff files reviewed indicated that each staff member did not submit documentation of a negative tuberculosis screening (TB) no later than 21 days after employment. Evidence - 1. On the date of inspection (04/14/2016), there was not an original TB documentation on file for - Staff #5, and Staff #6. 2. The listed start dates were - 02/09/2016 (for Staff #5), and 02/22/2016 (for Staff #6).

Plan of Correction: TB documentation shall be obtained for all applicable staff.

Standard #: 22VAC40-185-160-C
Description: Based on review, 1 of 7 staff files reviewed indicated that it has been more than two years from the date of the most recent TB documentation for a staff member. Evidence - 1. On the date of inspection (04/14/2016), the most recent TB documentation on file for Staff #2 was dated - 02/28/2014. 2. This finding was confirmed on the date of inspection by the Center Director.

Plan of Correction: Updated TB documentation shall be obtained for Staff #2.

Standard #: 22VAC40-185-70-A
Description: Based on review, 6 of 7 staff files reviewed indicated that not all of the required information was on file for each staff person. Evidence - 1. On the date of inspection (04/14/2016) educational documentation was not available for - Staff #1, and Staff #2 2. On the date of inspection, at least two completed references were not available for review for - Staff #3 (only 1 of 2), Staff #5 (only 1 of 2), Staff #6 (only 1 of 2), Staff #7 (no references of file).

Plan of Correction: All missing information shall be obtained and placed in the applicable staff file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, pieces of equipment both inside and outside were not maintained in a safe and/or operable condition. Evidence - 1. On the date of inspection (04/14/2016), a screen appeared ripped. It was located on an outer window that faced the pre-school playground. 2. On the date of inspection, a blind cord hanging within the Pre-K 2's room was not attached to its base and hung freely in place. It was located on the far left window along the back wall.

Plan of Correction: Both areas will be addressed and repaired.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as air fresheners, disinfectants were not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (04/14/2016), one spray can of air freshener, and one liquid spray bottle marked disinfectant were observed hanging from a shelf within the Beginners' bathroom. 2. Neither container was in use at the time of observation.

Plan of Correction: Items will both be relocated to a locked area.

Standard #: 22VAC40-185-510-C
Description: Based on review and interview, expired medication was observed on site. Evidence - 1. On the date of inspection (04/14/2016), medication on site, to be given on an as needed basis for Child #4, had the expiration date on it of January 2016. 2. The Center Director stated (during an interview conducted on the date of inspection) that new medication for Child #4 has already been obtained. The old medication has yet to be returned to the child's parent.

Plan of Correction: The old medication shall be returned to the Parent of Child #4.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review, 1 of 7 staff files reviewed indicated that it has been more than three years since the dates of the most recent central registry (CPS) finding and most recent criminal history record check (CRC) report. Evidence - 1. On the date of inspection (04/14/2016), the most recent CPS finding for Staff #1 was dated - 03/05/2012. 2. The most recent CRC report for Staff #1 was dated - 03/02/2012.

Plan of Correction: Updated request for a current finding and current report will be sent.

Standard #: 22VAC40-191-60-C-1
Description: Based on review, 1 of 7 staff files reviewed indicated that the center does not have an original criminal history record (CRC) report within 30 days of employment for all staff. Evidence - 1. On the date of inspection (04/14/2016), an original CRC for Staff #3 was not available for review. 2. The start date for Staff #3 was listed as 03/10/2016. No follow-up information was conducted.

Plan of Correction: The state police central criminal records exchange will be contacted to inquire status. If needed, a new request will be sent. Further employment shall be denied until acceptable documentation is obtained.

Standard #: 22VAC40-191-60-C-2
Description: Based on review and interview, 1 of 7 staff files reviewed indicated that the center does not have a central registry (CPS) finding within 30 days of employment or volunteer service. Evidence - 1. On the date of inspection (04/14/2016), an original CPS finding for Staff #3 was not available for review. The start date for Staff #3 was listed as 03/10/2016. Based on information written on a copy of what was sent; the request was not sent out until 03/21/2016. Staff #8 stated (during an interview on the date of inspection) that the request was returned to the center based on a lack of needed information, and was then sent out again. 2. On the date of inspection, an original CPS finding for Staff #6 was not available for review. The start date for Staff #6 was listed as 02/22/2016.

Plan of Correction: The Commonwealth of Virginia, Department of Social Services, central registry will be contacted to inquire status for both. If needed, a new request will be sent. Further employment shall be denied until acceptable documentation is obtained.
