Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated and completed on 7/11/2025, The on-site inspection began at 9:30am and ended at 10:50pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 18. The inspector reviewed 2 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/25/2025. A POC submitted after this date will not appear on the public website.

Description:	The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The inspection summary containing the violation notice from the most recent inspection, dated 12/14/2025, was not posted in a conspicuous place on the premises.

Description:	Providers are required to obtain a completed sworn statement prior to the employee's first day of employment The sworn statement for Staff #1 who has been employed for over 3 months, was not completed.

Description:	Providers must obtain a completed national criminal background check prior to the employee's first day of employment and when any new resident moves into the home. Staff #1, who has been employed for over 3 months, did not have a completed national criminal background check.

Description:	The provider must orient assistants by the end of their first week of assuming job responsibilities. Staff #1, who has been employed for over 3 months, did not have documentation of completed orientation within the first week of assuming job responsibilities.
