Comments:
An unannounced renewal study was conducted from 11:00 am - 1:15 pm. There were 109 children in care being supervised by 18 staff members. Lunch served today is pizza, mixed veggies, applesauce, and milk. The children were observed participating in story time and lunch time. Nine staff records were reviewed and in compliance. Ten children's records were reviewed and in compliance. Five emergency medications were reviewed and in compliance. The most recent emergency escape drill was conducted 04/13/2016 and the most recent shelter in place drill was conducted 03/01/2016. Standard #: 22VAC40-185-270-A
Description: Based on observation of the Toddler room it was determined that not all areas of the center were maintained in safe and operable condition. Evidence: The top of the changing table has a crack with sharp edges exposed.

Plan of Correction: The top part of the changing table was ordered on 03/16/2016 and is being shipped to the center today.
