During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated on 7/22/2025 and completed on 7/22/2025. The on-site inspection began at 10:20 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 10. The inspector reviewed 3 children?s records and 2 caregiver records on-site. This inspection included document review, a tour of the home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/26/2025. A POC submitted after this date will not appear on the public web

Standard #:	8VAC20-800-730-D
Description:	Medication for caregivers shall be kept in a locked place using a safe locking method that prevents access by children. Caregiver #1?s prescription medication was stored in an unlocked closet in the kitchen.
Plan of Correction:	Medication is already moved from kitchen closet.
Standard #:	8VAC20-800-750-A
Description:	When topical skin products such as diaper ointment are used, the product shall be labeled with the child's name. Child #1?s diaper cream was not labeled with the child?s name.

Standard #:	8VAC20-800-800-B
Description:	The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. The last annual review of the emergency plan was overdue by five months.

Description:	When food is brought from home, the food container shall be clearly labeled in a way that identifies the owner. Child #1?s dry formula container was not labeled in a way that identifies the owner. Child #2?s four bottles were not labeled in a way that identifies the owner. Child #3 `s food containers were not labeled in a way that identifies the owner.

Description:	Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregivers #1 and #2 obtained five training hours in 2024.

Description:	All flammable and combustible materials, such as oil and grease products, shall be stored in an area inaccessible to children. Olive oil was stored on the floor of the kitchen closet, accessible to children.
