An unannounced, on-site monitoring inspection was initiated on 7/23/2025 and completed on 7/23/2025, as a part of the licensure period. The on-site inspection began at 9:10am and ended at 10:10am. The inspector reviewed compliance in the areas listed above. There were three children present and two caregivers; the point total was four. The inspector reviewed three children?s records and three caregiver/household member(s) records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-200-C Description: Documentation of the orientation must be signed and dated by the provider and assistant.
Caregiver #3's record was missing their documentation of a signed and dated orientation form.Plan of Correction: Provider will add the documentation of a signed and dated orientation record.
