During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
MAT certification has expired, but there are currently no children with medication in care. First Aid and CPR certification will expire soon. Inspector will send MAT website to provider.

Comments:
An unannounced, on-site monitoring inspection was initiated on and completed on 7/11/2025. The on-site inspection began at 10:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were four children present and one caregivers the point total was ten. The inspector reviewed five children?s records and two caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: A child's record shall contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. There was no written authorization for emergency medical care for Child #5, who had been in care for one week.

Plan of Correction: The provider will obtain written authorization to obtain emergency treatment for the child.

Standard #: 8VAC20-800-50-A
Description: Providers must keep a written record of children in attendance each day. There were no attendance records for Child #5, who had been in care for one week.

Plan of Correction: The provider will begin taking attendance for the child.

Standard #: 8VAC20-800-60-B
Description: Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #5 was missing the second emergency contact. The child was present during the inspection and had been in care for one week.

Plan of Correction: The provider will obtain a second emergency contact for the child.

Standard #: 8VAC20-800-60-B
Description: Children's records are required to contain parental home and employment addresses and phone numbers. The record for Child #5 was missing the employment information for and the employment phone number for both parents. The child was present during the inspection and had been in care for one week.

Plan of Correction: The provider will obtain work phone numbers for both parents.
