A complaint was received at the Central Licensing Office on 4/18/16 regarding "physical plant" at the center. An unannounced complaint inspection was conducted on 4/20/16 by the licensing inspector regarding loose fence mesh at the center. Playground fencing was checked and documentation was provided by the facility indicating there had been a minor injury to a child relating to the loose fence mesh. Based on observation and information obtained during the inspection, the preponderance of evidence supports the complaint regarding the condition of the fence mesh in several areas. Therefore, the complaint is determined as valid. Approximate time on site for complaint: 1:15 pm - 2:15 pm

Description: Based on observation, the facility did not maintain fencing in a safe condition.
Evidence: The metal fence mesh on both the upper and lower playgrounds is detached from the ground level support bar. The mesh is loose enough such that a child's hand or foot could become entangled in it at ground level. The fence mesh on the infant/toddler playground (parking lot side) is also loose where it touches the ground and is not secured.

Plan of Correction: Corrective action has already been done . Zip ties were applied to the bottom of the fence to secure it and I have put in a request to the handyman. 4/13/16
