22VAC40-191 Background Checks (22VAC40-191)
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on April 20, 2016 from 10:30 a.m. through 3:00 p.m. On this date, there were sixty-one (61) children in the care of twelve (12) staff members in six classrooms. There are a total of sixty-five (65) children enrolled in this program. Five (5) children's records and fifteen (15) staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were outside play, library, transition, hand washing/bathroom and diapering procedures, lunch and nap. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Children's artwork is displayed throughout the classrooms. Standard #: 22VAC40-185-330-B Description: Resilient surfacing at the bottom of all three blue slides measured less than one inch in depth. Evidence: Based on measurement and discussion with the director it was confirmed the resilient surfacing at the bottom of the blue slides measured less than one inch in depth. Plan of Correction: The director has contacted the mulch manufacture and an order has been placed. Mulch will be added to ensure the measurement is six inches or greater in depth in the fall zones of all equipment.
Standard #: 22VAC40-185-510-E Description: One Epi Pen wasnot labeled with the child's name, the name of the medication, the dosage amount and the times or times to be given.
Evidence: Based on observation and discussion with the staff it was confirmed the Epi Pen was not labeled with the child's name, the name of medication, the dosage amount, and the time or times to be given.
Plan of Correction: The Epi Pen will be returned to the parent. The center will request an Epi Pen that has the pharmacy label attached.
Standard #: 22VAC40-191-40-D-1-C Description: Two staff records reviewed did not have documentation of obtaining an updated sworn statement, criminal history check and a central registry search. Evidence: Based on review of fifteen (15) staff records, Staff C and Staff I had not obtained updated background checks. Staff C's last documented criminal history check was dated 04/08/2013, sworn statement was dated 04/04/2013 and central registry search was dated 04/08/2013. Staff I's last documented criminal history check was dated 04/10/2013, sworn statement was dated 02/15/2013 and central registry search was dated 04/10/2013. Plan of Correction: The director will submit updated background checks on Staff C and I. In the future, updates will be repeated every three years from the previous date.
