During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements.

An unannounced, on-site monitoring inspection was initiated and completed on August 4, 2025. The on-site inspection began at 10:20am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 20. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records on-site. This inspection included interviews, observations, document review, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.036-B-1 Description: The home is required to obtain a completed sworn statement for any household members who are 18 years of age or older.
The records for Household Member #1 did not contain documentation of a completed sworn statement.Plan of Correction: Missing Sworn Statement will be completed ASAP.
Standard #: 8VAC20-800-170-A Description: The provider shall obtain from each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
There was no documentation of a tuberculosis screening for Household Member #1.Plan of Correction: Tb will be updated ASAP.
Standard #: 8VAC20-800-180-A Description: A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #1 was not completed and was overdue by six months.Plan of Correction: TB will be updated ASAP.
