22VAC40-191 Background Checks (22VAC40-191)

An unannounced inspection was conducted at the facility on April 20, 2016, from approximately 2:45 pm - 4:00 pm. Upon entrance there were 29 children and 7 staff . The Licensing Inspector observed children playing with manipulatives, waking up from nap, hand washing and story time. The Licensing Inspector reviewed 3 children records and 2 staff records. Medication is not given at the center. Licensing Inspector reviewed supervision, activities, equipment, hand washing procedures, nutrition, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on April 20, 2016, the licensing inspector observed next to the diapering area hand sanitizer. The hazardous substances was labeled "Keep out of reach of children" and "Warning."

Plan of Correction: Director spoke to the staff . Hand sanitizer will be kept locked cabinet in bathroom w/all other cleaning. Items staff was reminded to put away each time used.

Description: Based on observation, the license did not ensure children's hands were washed with soap and water after diapering. Evidence: A child in the two year old classroom was observed having a diaper change by staff. The staff member had the child to return playing in the classroom. The chid's hands were not washed after the diaper change. The staff member was waiting for sanitizer from other classroom.

Plan of Correction: Director spoke to the staff member and had her practice with other staff the proper procedures of diapering . Staff member froze up in front of the inspector . She knew what to do just very nervous.

Description: Based on observation, the licensee did not ensure the diapering surface was used only for diapering or cleaning children. Evidence: During the inspection conducted on April 20, 2016, the Licensing Inspector observed in the infant/toddler classroom baby wipes on top of the diapering surface.

Plan of Correction: Director spoke to all infant toddler staff and reminded them to put wipes beside the changing mat at all times . Never rest them on the mat area.
