During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced inspection was initiated on 07/23/2025 and completed on 08/04/2025 in response to a complaint received by the licensing office on 07/21/2025 relating to allegations of operational responsibilities and care of children. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 08/15/2025. A POC submitted after this date will not appear on the public website.

Description:	The provider shall ensure that the home's activities, services, and facilities are conducive to the welfare of children in care. Interviews revealed that staff #2 takes child #1, age 4 years and who is the biological child of staff #2, into the bathroom and spanks the child causing the child to cry.
Plan of Correction:	The staff member, who is also the parent of the child, has come up with other disciplinary consequences during school hours.

Description:	Physical punishment including striking a child shall be forbidden. Interviews revealed that staff #2 takes child #1, age 4 years and who is the biological child of staff #2, into the bathroom and spanks the child.
