Discussion of the events from April 14, 2016, that were self-reported to the Division of Licensing.

Comments:
The following inspection took place between the hours of # am/pm through # pm. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interview and review, the licensee did not ensure that the center's activities were maintained in compliance with these standards. Evidence - 1. Based on review of the Chesterbrook Academy Fair Lakes - Attention and Supervision Policy and Procedures (page 1) states: "One teacher should be in front of the line and should lead the children to the next classroom (area). The Second teacher should stand at the door and conduct a head count of the children as they are leaving the classroom. Once the last child has exited the classroom, the teacher at the door should reenter the classroom to look around and ensure that no child has been left behind in the bathroom or behind a shelf. When a classroom is closed, staff must ensure that the classroom is empty and all the children have transitioned along." Based on video, Child #1 was left in his classroom unsupervised for approximately 10 to 15 minutes. Neither Staff #1 or Staff #2 reentered the classroom to ensure that it was empty. Staff #1 and Staff #2 both took this training on 09/16/2014. Both have had additional trainings in supervision since that original date. 2. The Center Director stated (during an interview conducted on the date of inspection - 04/21/2016) that due to the fact that the playground is not immediately adjacent to the classroom, that a second Face to Name check must be done at the destination (or in this case 2's/3's playground). The teacher again stands in the doorway/threshold to the destination and then circles the original checkmark to show that the child reached the new site. Based on the written statements of Staff #1 and Staff #2, only a head count was conducted once they arrived at their destination.

Plan of Correction: Staff #1 and Staff #2 have been terminated. A retraining for all remaining staff shall be conducted on 04/28/2016.

Standard #: 22VAC40-185-340-A
Description: Based on interview and review of written statements, staff did not ensure the care, protection, and guidance of a child within their care. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection) that on 04/14/2016, at 4:35 pm., a child was left unsupervised in the 2 year old beginners classroom for approximately 10 to 15 minutes. 2. There were a total of 15 children with Staff #1 and Staff #2. The group was in transition from the classroom to playground. Both staff failed to follow policy and procedure with regards to supervision; a second name to face once they at the playground was not conducted. 3. The Grandfather of Child #1 found the child alone, and brought it to the attention of Director.

Plan of Correction: Staff #1 and Staff #2 have been terminated. A retraining for all remaining staff shall be conducted on 04/28/2016.

Standard #: 22VAC40-185-340-F
Description: Based on interview and review of written statements, children under 10 years of age were not always within actual sight and sound supervision of staff. Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection) that on 04/14/2016, at 4:35 pm., a child was left unsupervised in the 2 year old beginners classroom for approximately 10 to 15 minutes. 2. The child was not using the restroom at that time.

Plan of Correction: Staff #1 and Staff #2 have been terminated. A retraining for all remaining staff shall be conducted on 04/28/2016.
