Today we discussed first aid kits and nap time.
An unannounced monitoring inspection was conducted today between 9:40am and 12:50pm. There were 64 children in care with ten staff. The program director was also present. A tour of the facility inside and outside was completed. Children were observed in teacher and self-directed activities including but not limited to meals, outside play, inside play activities, transitions, restroom routines, diaper changing and hand washing. Medication, fire inspection and health inspection were reviewed. Violations related to posted emergency evacuation routes, unlocked chemicals, first aid kit, and staff orientation were cited and may be viewed on the violation notice. Upon receipt of the inspection documentation, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: ? The steps to correct noncompliance with the standard(s); ? Measures to prevent reoccurrence of noncompliance; ? Person(s) responsible for implementation and monitoring of preventive measure(s); ? Date by which noncompliance will be corrected. The licensee will have ten calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office by 4/25/16. The licensee should retain a copy to be posted at the facility (Supplemental Information is not to be posted due to confidentiality). Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 15 calendar days, regardless of whether the Plan of Correction section is completed.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview the center failed to ensure that each staff record contains all required information. Evidence: 1 - The record for staff 2 does not contain documentation that all portions of the required orientation was completed. 2 - Staff 1 confirmed that the documentation was not present in the record during the inspection. Standard #: 22VAC40-185-280-B Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method. Evidence: 1 - The teacher closet in the two year old classroom contains a spray can of disinfectant. The closet was unlocked during the inspection. The label on the spray can reads keep out of reach of children and hazardous. 2 - The key to the teacher closet was in the lock in the unlocked position. 3 - Staff 1 confirmed that the key was in lock. Staff 1 stated that should not be in there it should be up here [on top of the cabinet]. 4 - In the school age room there is a bottle of rubbing alcohol and two cans of shaving cream. The label on the containers read keep out of reach of children and harmful and fatal. 5 - Staff 1 confirmed that the items were stored in an unlocked location. Standard #: 22VAC40-185-370-1 Description: Based on observation and interview the center failed to ensure that when an infant is placed in his crib, he shall be placed on his back (supine). Evidence: At 9:53am staff 2 placed 12 month old infant 1 in her crib on her stomach and began patting her back. The staff then walked away from the crib leaving the sleeping child on her stomach. Evidence: 1 - The first aid kit in building 2, the school age building does not contain triangular bandages. 2 - Staff 1 confirmed that the there were not triangular bandages in the first aid kit in building 2.
