Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated on 08/06/2025 and completed on 08/06/2025. The on-site inspection began at 11:15 am and ended at 1:05 pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 caregivers; the point total was 20. The inspector reviewed 7 children's records and 2 caregiver records on-site on 08/06/2025. This inspection included: *Document Review *Tour of the child care areas of the home *Interviews *Observations Information gathered during the inspection determined non-compliance with applicable standards and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-800-180-A
Description:	The provider shall obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening. The tuberculosis screening for caregiver 1 and caregiver 2 was completed in June of 2023.
Plan of Correction:	The provider stated she misread the information on the screening form which she thought indicated the screening was good until October of 2025. An updated TB screening was obtained for caregiver 1 and caregiver 2 immediately following the inspection.
