The Licensing Inspector conducted an investigation in response to a self report incident of a staff hitting a child on 3/24/16. A joint investigation with another agency was completed on 3/29/16, the chid was interviewed. The alleged staff was not available for interview this date. On 4/15/16 the alleged staff failed to attend an interview that was scheduled jointly with another agency. On 4/15/16 the other staff member working in the classroom during this alleged incident was interviewed at the center by the LI. The information gathered during this investigation supports the allegations; violations were discussed with the center director. Standard #: 22VAC40-185-410-1 Description: Based on interviews, staff striked a child on the hand. Evidence: Staff #1 was not available for interview by this Licensing Inspector, in the staff's written statement she stated that on 3/24/16 she had asked the child to put away the play dough for lunch. The child would not so the staff took the playdough from the child's hand. According to staff #1 the child started yelling and kicking. Staff wrote in the statement that "she get frustrated with his behave so I get his hand to try to stoped". Staff #2 stated that the child got upset when the play dough was taken from him. Staff #2 stated, "I started putting down the cots around the same time the child's mom came in and saw staff #1 hit the chid". Staff #2 did not see the other staff hit the child. The staff stated that staff #1 explained "I just "lightly tapped him on the hand because he was throwing his tantrum for 30 minutes plus". Staff # 1 was interviewed on 4/19/2016 by another agency where she admitted to tapping the child on his hand. Plan of Correction: Staff #1 was terminted and all staff were retrained on positive guidance.
