An unannounced, on-site renewal inspection was conducted on 8/12/2025 from 12:20pm - 1:35pm. There were 4 children and 1 caregiver present during the time of the inspection; totaling 3 points. The inspector reviewed 3 child records and 4 caregiver/household records onsite. The inspection included document review, interviews, observations, and a walk thru of the program.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 8/26/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.036-B-3 Description: The provider must obtain the results of a central registry search (CPS) for each household member within 30 days of the first day of residence.

The record for Child #1, enrolled for more than 2 months, did not contain employment information for 1 parent or the policy number of their medical insurance. Before the child's first day of attendance, parents shall be provided required information in writing.
The parents of Child #1, enrolled for more than 2 months, and Child #2, enrolled for 7 months, signed the Child Information form; however, it was not completed by the provider and did not provide the following: schedule of fees and payment plans, hours and days of operation, holiday closures, discipline policies, provision of food, general daily schedule, policies for termination of care, name and schedule of assistant/subsitute provider, and provisions of the family day homes emergency preparedness and response plan.Plan of Correction: Not available online. Standard #: 8VAC20-800-90-A Description: Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia and State Baord of Health.
