Thank you for your time and assistance with the unannounced monitoring inspection conducted on May 9, 2016. The on-site inspection was from 11:25 a.m. to 3:35 p.m. On this date, at the time of arrival, there were twenty (20) children in the care of three (3) staff members and one (1) extra staff member was available to assist, if needed. There were twenty-three (23) children enrolled in the program. There were six (6) staff records checked for updates and four (4) children?s records reviewed. There were no medications being administered at the time of the inspection. There were four (4) staff members and two (2) children interviewed, and three (3) classrooms were observed. Due to inclement weather, the playground was not observed. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. Lunch consisted of hot dogs, buns, broccoli, honeydew melon, and milk. Choice time, infant feeding, infant care, diaper changing procedures, hand washing and bathroom procedures, lunch, nap time, afternoon snack, and dismissal procedures were observed during the visit. Other items reviewed included required posted information, the first aid kit and emergency supplies, evacuation and shelter-in-place drills, annual fire and health inspections, and injury reports. Description: Based on observation and verification from the director on 05/02/2016, the center failed to keep hazardous substances in a locked place that prevents access by children. Evidence: 1. In each of the two bathrooms used by the children in Classroom B and Classroom C, there was a spray bottle of bleach water in an unlocked cabinet. 2. The director verified that the locking mechanisms used on the cabinet doors were not functioning properly and the doors were not locked.

Plan of Correction: During the inspection, a staff member went to the hardware store and purchased new locks for the cabinets containing hazardous substances. They were being installed during the inspection.

Standard #: 22VAC40-185-320-B
Description: Based on thermometer readings and discussion with the director on 05/02/2016, the temperature of the water from the sink in one bathroom measured above 120 degrees. Evidence: 1. The water temperature at the sink used by the children in Classroom B and Classroom C was measured to be 124.5 degrees.

Plan of Correction: The hot water heater had just been replaced and immediately during the inspection, the director turned down the temperature of the hot water heater. Within two hours the temperature of the water had dropped to 96.7 degrees. The staff will continue to monitor the temperature of the hot water and adjust the hot water heater accordingly.
