An unannounced monitoring inspection was conducted on May 2, 2016 from approximately 4:00 pm to 5:50 pm. Two staff were present with twenty-six children at arrival to facility. Additional staff and children arrived during inspection. Children were observed playing large group games in the gymnasium and eating dinner in the cafeteria. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the Site Supervisor during the exit interview conducted on this date. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-550-D Description: Based on review, facility did not ensure evacuation drills are conducted monthly. Evidence: On May 2, 2016, there was not documentation of a completed evacuation drill conducted in April 2016. Plan of Correction: The Center Supervisor will ensure all evacuation drills are documented once complete.
Standard #: 22VAC40-185-550-M Description: Based on review, facility did not ensure 2 of 2 injury reports have documentation of required information. Evidence: On May 2, 2016, injury reports completed on February 12, 2016 and April 11, 2016 did not document the time of the injury. Plan of Correction: The Center Supervisor will ensure the accident/incident log is filled out completely.
Standard #: 22VAC40-185-560-F Description: Based on review, facility did not ensure when food is provided there is a posted menu showing meals and snacks served for the current week. Evidence: On May 2, 2016, the menu posted was dated for April 2016. May menu was posted during inspection however it stated hot dog, potato rounds, fruit and milk were to be served on this date. Children were observed eating chicken nuggets, broccoli, apple and milk. Plan of Correction: The Center Supervisor will ensure the menu posted is of the current week.
