Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 8/19/2025. The on-site inspection began at 11:45 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was seven. The inspector reviewed three children?s records and one caregiver record on-site. This inspection included: ? document review ? tour of the facility ? interviews ? observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/02/2025. A POC submitted after this date will not appear on the public website.

Description:	The provider shall keep a written record of children in attendance each day. There was no attendance record for the current date of the inspection.

Description:	The provider shall obtain documentation that each child has been adequately immunized before the child's first day of attendance. There was no documentation of an immunization record on file for child A, who had been attending the program for one day.

Description:	Potentially poisonous substances, materials, and supplies shall be inaccessible to children. Several bottles of shampoo, conditioner, and other health and beauty products that were labeled "keep out of reach of children" were observed in the shower of the bathroom located on the same floor as the day care area, accessible to children.

Description:	Children under two years of age and children over two year of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs. There was no gate in use to prevent access to the staircase. A child under two years of age was observed walking in the hallway and had access to the stairs.
