An unannounced monitoring inspection was conducted from 9:00 am-11:30. At the time of entrance, 142 children were in care with 26 staff members present. Children were observed engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-160-A Description: Based on record review, licensee has not maintained documentation of negative tuberculosis screening for staff no later than 21 days after employment. Evidence: Staff #1, hired 1/20/16 and staff #2, hired 3/31/16 have not submitted documentation of a negative tuberculosis screening within 21 days of their hire. Plan of Correction: Staff #1 last day is today, 5/9/16 and staff #2 will have his current TB results faxed over to the center today.
