22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
Due to a self-reported incident by the facility received in the Valley Licensing Office May 13, 2016 and investigation was conducted on this date. Documentation was reviewed and interviews were conducted. There was a preponderance of evidence that there was an error made in documentation. Any violations are identified on the violation notice. Standard #: 22VAC40-185-510-L Description: Based on documentation and interviews the facility failed to appropriately document the staff member administering a medication. Evidence: 1. On 05/13/2016 the licensing inspector reviewed documentation and there was an error made in the documentation of the staff member who administered one dose of a medication for child #1. 2. On 05/13/2016 staff #1 and staff #2 confirmed there had been an error made in the documentation of the administration regarding one dose of medication for child #1. Plan of Correction: We were able to present to the licensing inspector exactly who the member was that administered the one dose of medication for the child. We will review this with all MAT trained staff in order to ensure it doesn't happen again.
