Standard #: 22VAC40-185-160-A
Description: Based on a review of seven staff records, it was determined that the facility did not ensure that each staff member has documentation of a negative tuberculosis screening within 21 days after beginning employment. Evidence: 1. The record for staff #4 (date of hire 10/26/15), working during the inspection, did not contain documentation of a negative tuberculosis screening. 2. Staff #7 (Program Director) confirmed that staff #4 did not have documentation of a negative tuberculosis screening.

Plan of Correction: Staff #4 will complete a TB screening. All new staff will complete a TB screening within 21 days of employment.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that the center maintains and keeps at the center a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #1 (date of enrollment 12/7/15), present during the inspection, did not have documentation whether or not the child previously attended child day care. 2. The record for child #2 (date of enrollment 3/28/16), present during the inspection, did not contain the contact and employment information for the father. 3. The record for child #3(date of enrollment 2/22/16), present during the inspection, did not contact the contact and employment information for the father, and did not have documentation whether or not the child previously attended child day care. 4. The record for child #1 (date of enrollment 12/7/15), present during the inspection, did not have documentation whether or not the child previously attended child day care. 5. Staff #7 (Program Director) reviewed the records for child #1, child #2, child #4, and child #5 and confirmed that they did not contain all of the required information.

Plan of Correction: The parents will be asked to provide the missing information for each child. Records will be reviewed on a regular schedule to ensure all are complete.

Standard #: 22VAC40-185-70-A
Description: Based a review of seven staff records, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence: 1. The record for staff #4 did not contain documentation of two or more references as to character and reputation as well as competency were checked before employment. 2. Staff #7 (Program Director) confirmed that there were no references available for viewing during the inspection for staff #4.

Plan of Correction: References will be completed and placed in the record for staff #4. New staff will not be allowed to begin employment until their references are completed.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of seven staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #4 (date of hire 10/26/15), working during the inspection, did not contain documentation of a completed search of the central registry finding. 2. Staff #7 (Program Director) confirmed that the search of the central registry finding has not been received for staff #4.

Plan of Correction: A new search of the central registry for staff was mailed on 5/6/16 and we are waiting to receive it.
