22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:00am-10:45am. Upon arrival there were 127 children in care with 22 staff present. A sample of 4 children and 5 staff records were reviewed in addition to 6 medications. Children were observed eating morning snack, on the playground and during a golf lesson by an independent contractor. The center provided transportation and the first aid kit on the center's vehicle was inspected.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: 1. In the hallway there were 4 mat used for napping that were torn in the corners, exposing the inside foam. 2. In the 3 year-old classroom there were 2 large plastic bins on a shelf over the children's toilet. The bins did not sit flush on the shelf and hung over the edge. When touched the bins and the shelf gave way and was unstable. Plan of Correction: The mats were repaired during the inspection and the plastic bins were moved to another location on the floor.
