- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection May 19, 2016 from approximately 10:00am to 1:00pm. The following ratios were observed with a total of 31 children present during the inspection:
Threes (Pre-K 3) ? 13 children with 2 staff
Fours & Fives (Pre-K 4) - 8 children with 2 staff
Fours & Fives (Pre-K 5) - 10 children with 1 staff
School Age children were not present during the inspection.
During the inspection, the Pre-K 3 classroom was observed playing outside. The Pre-K 4 classroom was observed participating in circle time where they reviewed the number six. Later the children completed worksheets reviewing the number 6 and practicing their counting skills. The children then washed hands and ate their snack they brought from home. Later the Pre-K 4 classroom went outside to play. The Pre-K 5 classroom was observed participating in small groups where they learned about different types of transportation. The children created and colored boats, practiced their writing and cutting skills, and created transportation books. Later the children were observed playing outside. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: health and fire inspections, emergency supplies, children?s assessments, fire and shelter-in-place drills, daily attendance, parent agreements, and staff training information. Management stated medication is not administered at this time.

Standard #: 22VAC40-185-160-A Description: Based on review of three staff records, one staff record did not contain documentation of a negative tuberculosis screening within the required time frame.
During the inspection, the licensing inspector observed the record for Staff #2 (hire date 11/04/15) did not contain documentation of a negative tuberculosis screening. Documentation of a negative tuberculosis screening is required to be obtained 12 months prior to or within the first 21 days of beginning employment.
Plan of Correction: Per management - I will work with the staff to obtain this information.
Standard #: 22VAC40-185-70-A Description: Based on review of three staff records, three records did not contain the required information.
During the inspection, the licensing inspector observed the following:
1. The records for Staff #1 (hire date 09/01/15), Staff #2 (hire date 11/04/15) and Staff #3 (hire date 09/01/15) did not contain documentation of orientation.
2. The records for Staff #1 (hire date 09/01/15) and Staff #2 (hire date 11/04/15) did not contain documentation of at least two character references that were obtained prior to staff beginning employment.
3. The records for Staff #3 (hire date 09/01/15) did not contain a completed criminal history record check and the address of the designated emergency contact.
Plan of Correction: Per management - I will work with the staff to obtain these items. I will also check my records for the missing criminal record check. If I cannot locate it, I will contact the local agency to get a copy or obtain another one.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the facility did not have the most recent inspection findings posted on the premises. Evidence: During the inspection, the licensing inspector observed the inspection finding notice posted was dated 04/29/15. An inspection was conducted on 11/20/15 and the inspection finding from that inspection was required to be posted. Plan of Correction: We will post the inspection finding from today.
