Comments:
Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on May 19, 2016 from 11:15 a.m. through 2:00 p.m. On this date, there were one-hundred seventeen (117) children in the care of twenty-five (25) staff members in fourteen classrooms. There are a total of one-hundred thirty-eight (138) children enrolled. Five (5) children's records and five (5) staff records were reviewed and updated. The medication and medication paperwork was reviewed for two children. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were outside play, free choice activity, hand washing/bathroom and diapering procedures, infant feeding, lunch and nap. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Description: Child B did not have documentation of having a physician's phone number and a home address for parents. Evidence: Based on record review and conversation with the provider it was determined Child B did not have documentation of a physician's phone number and a home address for parents.

Plan of Correction: The director will request the parent furnish documentation of the physician's phone number and home address.
