Comments:
The licensing inspector conducted an unannounced monitoring inspection Friday, May 13, 2016 from 11:06am to 1:19pm. The children were observed eating lunch and getting ready for rest time. The infants were observed playing on the floor and in bounce chairs. A formal observation was completed with the preschool class. They were eating lunch, which consisted of baked ziti with meat sauce, carrots, apples and milk. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Four children?s records and four employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill:4/22/2016 Last shelter-in-place drill: 2/25/2016 Last fire inspection: 12/30/2015 Last health inspection: 1/8/2016 Total enrollment today: 52 Today, the following child to staff ratios were observed: Infants ?10:3 Toddlers ?8:2 Twos ? 14:2 Threes ?13:2 Pre-K ?7:1 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-570-E
Description: Based on observation on 05/13/2016, the center did not ensure that bottles for infants were dated. EVIDENCE: The bottles of formula for Child #5 were not dated. Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

Plan of Correction: Corrected on site. Bottles dated.
