Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated and completed on September 3, 2025, as a part of the licensure period. The on-site inspection began at 11:00 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 16. The inspector reviewed 4 children?s records and 2 caregiver/household member(s) records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 10, 2025. A POC submitted after this date will not appear on the public website.

Description:	Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The record for Child #3 (enrolled for over 10 months) was missing the addresses of two designated emergency contacts.

Description:	Documentation of additional immunizations is required once every six months for children under the age of two years. Child #1, (12 months old), last documented immunization was over 7 months ago and overdue by over 1 month. Description:	Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. The bottom of the bathroom door in the children's play area was not in a safe condition when there was chipping wood present, accessible to children, that was a hazard to the children in care.

Description:	Small electrical appliances shall be unplugged unless being used by the caregiver or with children under close supervision. There was a water kettle plugged in in the upstairs kitchen and not being used.
