An announced record review was conducted on May 18, 2016. A sample of three staff records were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the Recreation Operations Coordinator during the exit interview. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-70-A Description: Based on a record review in three of three records reviewed, the licensee did not ensure each staff record contained all required documentation. Evidence: During the inspection conducted on May 18, 2016 the following records were determined to be incomplete; 1) The record for staff #1 did not include the written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position. 2) The record for staff #2 did not include the documentation of information about any health related problems that may interfere with fulfilling the job responsibilities. 3) The record for staff #3 did not include the documentation of the complete address of a person to be notified in the event of an emergency, and information about any health related problems that may interfere with fulfilling the job responsibilities. Plan of Correction: Received transcript for staff #1. File updated. The emergency contact forms will be completed and files will be updated.
Standard #: 22VAC40-185-240-C Description: Based on a record review in one of three records reviewed, the licensee did not ensure each staff who work directly with children obtained 16 hours of annual training activities that are related to child safety and development and the function of the center. Evidence: During the inspection conducted on May 18, 2016 the record for staff #3 included documentation of obtaining only 12 of the required 16 hours of annual training. Plan of Correction: Training hours received outside of JCC will be requested from employee.
