22VAC40-191 Background Checks (22VAC40-191)
An announced record review was conducted on May 18, 2016. A sample of three staff records were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the Recreation Operations Coordinator during the exit interview. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-160-C Description: Based on a record review in one of three records reviewed, the licensee did not ensure each staff submitted documentation of a negative tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing. Evidence: During the inspection conducted on May 18, 2016 there was no documentation of an updated TB screening for staff #3, as the most recent screening is dated 11-22-2013. Plan of Correction: TB screenings was received on 5-23-16. File will be updated.
Standard #: 22VAC40-185-70-A Description: Based on a record review in one of three records reviewed, the licensee did not ensure each staff record contained all required documentation. Evidence: During the inspection conducted on May 18, 2016 the record for staff #3 did not include the documentation of the name, address and telephone number of a person to be notified in the event of an emergency, and information about any health related problems that may interfere with fulfilling the job responsibilities. Plan of Correction: The emergency contact form will be completed and file will be updated.
