During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. * Discussion that for programs subject to national fingerprint background checks pursuant to ? 22.1-289.035 of the Code of Virginia, VDOE will be unable to continue to cover the cost of national fingerprint background checks beyond December 31, 2025. Effective January 1, 2026, child day programs and family day systems will be responsible for covering the cost of obtaining fingerprint-based background checks. Child day programs and family day systems are encouraged to plan for this change. When preparing for operating costs beyond December 2025, fingerprint fees for employees, applicants, agents, caregivers, approved providers in a family day system, and household members will be $59.00 per individual request.

An unannounced, on-site monitoring inspection was initiated and completed on 09/02/2025. The on-site inspection began at 9:45 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (4) children present, plus 1 resident child, and (2) caregivers; the point total was (15). The inspector reviewed (4) children?s records, (2) caregivers records, and (3) household members records on-site on 09/02/2025.
This inspection included: document review (i.e. Files, injury logs, policies and procedures, and emergency drill logs); tour of the facility; interviews; and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-780-B Description: Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
Records for children #1, #2, and #3 did not have a current (within the past year) parent?s signature for an annual review, verifying their child's emergency information.Plan of Correction: This information will be verified by each child's parent.
Standard #: 8VAC20-800-120-B Description: Assistant records shall contain an emergency contact.
An emergency contact for Staff #2 was not available for review.Plan of Correction: This information will be obtained and placed on file.
