During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 09/11/2025 and completed on 09/11/2025. The on-site inspection began at 1:00 pm and ended at 2:00 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers;
the point total was 18. The inspector reviewed 6 children?s records and 8 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
09/19/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.011-F
Description: The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The inspection summary and violation notice from 03/07/2025 was not posted at the family day home.

Plan of Correction: The inspection notice was printed and on file but not posted on the board. It is now posted in the appropriate location.

Standard #: 8VAC20-800-800-B
Description: The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

The annual update for the emergency plan was overdue by 1 month.

Plan of Correction: The emergency plan has been reviewed and updated for the next year.

Standard #: 8VAC20-800-80-C
Description: The provider shall document in the child's record: The method of verification of the child's age and identity.

Child #1 has been enrolled for more than 3 months and did not have documentation of the method of verification of the child's age and identity.

Plan of Correction: Although I saw the document at registration, I inadvertently failed to complete the section regarding birth records at that time. The records were reverified and all information recorded.
