63.2 Facilities and Programs..
On 4/21/16, an unannounced inspection was conducted in response to a self reported incident regarding supervision. Staff interviews were conducted and records reviewed. A violation was found and a Violation Notice was issued. If you have any questions, I can be reached at 703-479-4709.
Standard #: 22VAC40-185-340-F Description: Based on interviews, on 3/31/16, staff did not ensure the supervision of an eight year old child with special needs when the child ran out of the classroom and out of the building and ran towards his home. Staff stated they saw the child leave the classroom and pursued him, but lost visual contact and notified the police. The child was found by the police near his home and returned to the center approximately 20 minutes later. Plan of Correction: Staff will ensure sight and sound supervision of each child maintained at all times and will monitor the doors at the center to ensure they are kept closed. Additional training will be provided.
