During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 9/22/2025. The on-site inspection began at 11:18am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 4 caregivers; the point total was 32. The inspector reviewed 2 children?s records and 4 caregiver/household members records on-site on 9/22/2025.
This inspection included document review, tour of the home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-500-C Description: Stationary outdoor playground equipment shall not be installed over concrete, asphalt, or any other hard surface; be placed at least six feet from the perimeter of other play structures or
obstacles; and be firmly anchored with ground supports that are covered with materials to
protect children from injury.
The provider had stationary outdoor playground equipment that was placed less than 6 feet from the perimeter of the fence and other obstacles.Plan of Correction: We will move it to the standards.
