Comments:
An unannounced monitoring inspection was conducted today, 6/1/2016. Upon arrival to the center the school age class was preparing to go on a field trip to a movie theatre. The two's and three's class were busy with the table toys and reading books. The classes then went outside to play on the playground. Diapering was also observed. You are reminded that when water tables are used outside they should be empty daily to prevent mosquitoes.
There are 35 children in care ranging in age from 2 yrs to 10 yrs of age with six staff.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the playground during the inspection the center failed to maintain the area in a safe manner. Evidence: 1. The blue plastic panels on the wooden climber/slide is broken and could cause a pinch point. This area is accessible to the children. 2. The plastic covering on the swing chains is peeling off. This is accessible to the children.

Plan of Correction: The plastic panels will be replaced or repaired. The plastic covering will be removed. The staff will monitor these areas to ensure compliance.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground during the inspection the center failed to keep the fall zones free of equipment. Evidence: 1. The Little Tykes slide/climber is placed next to the swing, in the fall zone.

Plan of Correction: The Little Tykes equipment will be placed in an area that will not be in the fall zones of equipment.
