Thank you for your time and assistance with the unannounced monitoring inspection conducted June 2, 2016. The on-site inspection was from 10:50 a.m. to 3:00 p.m. On this date, at the time of arrival, there were sixty-four (64) children present in seven classrooms. There were a total of eighty-nine (89) children enrolled in the program. There were ten staff members present and two extra staff members available to assist if needed. Seventeen (17) staff members' records were reviewed and seven children's records were reviewed. The medication and medication paperwork were reviewed for four children. There were eight staff members and three children interviewed. The exception to compliance was noted on the violation notice and the risk rating for the violation was documented on the supplemental page. Lunch consisted of chicken patties, broccoli, mixed fruit, and milk. All interactions between the staff and the children were age appropriate and positive. Choice time, hand washing and bathroom procedures, story time, outdoor play, lunch, nap time, and dismissal procedures were some of the activities observed during the visit. Standard #: 22VAC40-191-40-D-1-C Description: Based on the review of 17 staff records and verification from the director, the center failed to obtain an updated background check for one staff member. Evidence: 1. The record for Staff Member M did not contain a completed search of the central registry within three years since the date of the last central registry finding. 2. The date of the last central registry finding for Staff Member M was 05/01/2013. Plan of Correction: The director sent for the search of the central registry on 05/09/2016 and is waiting for it to be returned. In the future, she will ensure that the requests are mailed early enough so that they can be completed and returned within three years of the most recent finding.
